Qualifications:
Two to three years' experience in social service delivery with an elderly and disabled population.
Must have a working knowledge of supportive services and other resources in the geographic area serviced by the Property Management Company.
Bachelor of Social Work or degree in Psychology or Counseling preferred.
Company vehicle provided for properties visits, properties located in Dodgeville, Prairie du Chein, Westby, Desoto & Wauzeka Duties:
Assist elderly individuals and persons with disabilities, living in federally-assisted multifamily housing, to obtain needed supportive services from community agencies.
Services are intended to assist tenants to continue living independently.
Including:
Intake, Education, Connecting with Services Available, Application Procedures and referral of tenants to service providers in the general community.
Sponsor educational events which may include health care, agency support, life skills, referral sources, etc.
Monitor ongoing provision of services from community agencies.
Serve as a liaison to community agencies, network with community providers and seek out new services available to tenants.
Distribute materials from organizations who provide services to the clientele.
Work with Property Management team to seek solutions regarding tenant/management related issues.
Establish links to community agencies and services providers.
Assist tenants in obtaining needed equipment, devices, to help them live a more independent lifestyle.
Assist tenants in understanding the rules and regulations required to live in the apartments and to live with a diverse group of tenants.
Complete all paperwork required by the Service Coordinator Program department of HUD.
Must have an active driver's license.
Travel required with the use of company car.
